_OnStar Fleet Master Account Service Subscription Agreement-Canada--

This site is for processing the OnStar Fleet Master Account Service Subscription Agreement, which
includes the OnStar User Terms, Privacy Statement and Software Terms (the “Agreement”) between
the Fleet customer and General Motors of Canada Company, operating as OnStar.

To continue with the acceptance process, please follow these steps:

1. Review a copy of the Agreement by clicking “HERE” including the OnStar User Terms, Privacy Statement and Software
Terms available at www.onstar.ca.

2. Then, provide the information requested below, select both ‘AGREE’ check boxes and click on “Launch Electronic Signature
Process”. If you do not select both ‘AGREE’ check boxes, you will not have access to OnStar services in your fleet’s vehicles.

3. You will then be asked to consent to complete the transaction electronically. To proceed you must click "YES".

4. You will then be taken to a personalized copy of the Agreement to review and E-Sign. To E-Sign the Agreement, scroll down
and click on the "CLICK HERE TO SIGN" button. An acknowledgement page will confirm your completion of the signing process
and a copy of the executed Agreement will be sent to the email address you provided.
